When registering, please prepare your full name and Polish passport expiry date.

The consul cannot make copies of the documents.
REMINDER
- Persons whose passports expired before 2013 are obliged to submit a document confirming their Polish citizenship (to be obtained at the voivodship office in Poland). Both the original document and its copy should be submitted (a copy will remain on the Consulate’s files).
- In the case of passports issued after 2003, also passports for children which validity has already expired or will expire soon, you can apply for a new passport without the need to confirm Polish citizenship, but you must present an additional valid photo ID confirming the applicant's personal data (e.g. Polish ID card, an Australian passport).
- Persons who have changed their marital status and / or personal data are obliged to submit relevant decisions of the Registry Office in Poland. Both the original documents and their copies should be submitted (copies will remain on the Consulate’s files). Foreign documents are not accepted.
- Both parents submitting passport applications on behalf of their minor children are required to present their valid ID documents, child’s birth certificate and a valid child’s photo ID. Children from 5y.o. onwards need to be present when submitting Polish passport application. Both the original documents and their copies should be submitted (copies will remain on the Consulate’s files).
