The registration for passport duty will be accepted only on October 13th and 14th, 2025.

by phone from 10:00 a.m. until 1:00 p.m. at the telephone number (02) 9302 5000; or

by e-mail using the form (attached below) sent to: sydney.visa.passport@msz.gov.pl

 

Please remember to prepare the following information when registering for your passport appointment:

  • personal information (first name, last name, PESEL number, or if you don't have a PESEL number, date and place of birth),
  • contact information (telephone number),
  • details of your last current Polish passport or ID card;

Please ensure that:

  1. if your child was born outside of Poland, the foreign birth certificate was transcribed (registered) in Poland. Only after obtaining a copy of the Polish birth certificate can you apply for a passport for your child.
  1. if you got married outside of Poland, the foreign marriage certificate was transcribed (registered) in Poland. Only after obtaining a copy of the Polish marriage certificate with the name of the spouse can you apply for a passport;
  1. in the case of change of name, such change has been registered in Poland.

 

NOTE: Number of places for the passport duty is limited. Registration will follow until all places have been used.

Applications from the disabled people, people over 70 years, and parents/carers of small children will be considered first.

 

REMINDER

  • Persons whose passports expired before 2013 are obliged to submit a document confirming their Polish citizenship (to be obtained at the voivodship office in Poland). Both the original document and its copy should be submitted (a copy will remain on the Consulate’s files).
  • For expired passports, you must also present a valid photo ID confirming the applicant’s personal details (e.g., Polish ID card, Australian passport). Personal detail in both documents should be the same.
  • Parents submitting passport applications on behalf of minor children are required to present their valid ID documents, including at least one valid Polish passport and the child's Polish birth certificate. Both parents and the child aged over 5 or over must be present when submitting
    a passport application for a child.

Fees can be paid during the visit in cash, by payment card, or by direct transfer to the account of the Consulate General of the Republic of Poland in Sydney.
