Consulate General of the Republic of Poland in Los Angeles informs that a consular outreach will be held at the Polish Home Associated in Seattle, WA on October 24-25, 2025. Appointments during the visit will be held solely for the purpose of submitting passport applications.

IMPORTANT! Appointment dates will be made available on the website above on Sunday, October 5, 2025, at 6:00 PM (PDT / Los Angeles time).
REGISTRATION RULES:
- After opening the above-mentioned page, select "Consular Duty Appointment in Seattle, WA (October 24-25, 2025)" in the "Service Type" field, then complete all required fields and follow the instructions on the page.
- NOTE: After successfully scheduling your appointment, please send the system-generated appointment confirmation along with a copy of a valid ID to the following email address: la.dyzury@msz.gov.pl.
- For a child's first passport, please attach a scan of the child's Polish birth certificate (the original must be brought to the appointment).
- For an adult's first passport, please attach a scan of the decision issued by the voivode confirming Polish citizenship, a Polish birth certificate, and a Polish marriage certificate (for individuals married abroad). (The original documents must be brought to the appointment).
- FEES: Payments are accepted only in the form of money orders or cashier's checks (we do not accept cash or personal checks) made payable to the Consulate General of Poland in Los Angeles.
- NOTE: You must bring a current passport photo to your passport appointment – taken no more than six months before submitting your application. Failure to do so will result in your application not being accepted at your scheduled appointment. Photo requirements can be found at:

- Parents should bring their valid Polish ID documents to apply for a passport for their minor child. At least one parent should present a valid Polish ID (passport or ID card).
