Consular passport outreach in Greenacres (Miami), FL (December 12-13, 2025)

Next consular outreach organized by Polish Consulate in Houston will take place on December 12-13, 2025, at the following address:

Only passport applications will be accepted. Each appointment is for one passport application only and must be registered through the e-consulate system. The Consulate does not accept appointment registration by email or phone. No walk ins please.

Registration for consular outreach starts on December 1, 20225 at 11:00 am (CST) at: https://secure.e-konsulat.gov.pl/placowki/235. First time applicants who do not have a PESEL number shall register by selecting the type of visit:

    1ST PASSPORT 5 OR 10 YEARS  - PESEL NOT ASSIGNED

IMPORTANT! When registering, please provide a CORRECT TELEPHONE NUMBER and E-MAIL ADDRESS – a member of our staff will contact you to arrange the details of your appointment after the registration is completed. Please wait patiently for us to contact you. Appointments for persons who cannot be reached will be canceled.

Before booking an appointment for passport matters, please check that you have all the necessary documents. Detailed information on passport matters can be found at: https://www.gov.pl/web/usa/paszporty

If your Polish passport already has expired, please prepare another valid government issued photo ID, such as a US passport or driver's license. All changes in personal data (e.g., change of name/names in the United States or marital status resulting in the change of name) shall be registered in Poland PRIOR to the appointment. If you do not have Polish civil status records or decision confirming these changes, please do not make an appointment or cancel one you already have. These changes has to be  registered in the Polish civil status register for passport application eligibility. We accept applications for civil status registration BY MAIL.

We reserve the right to cancel the appointment in case of personal data discrepancy or a long gap from the date of expiration of the previously held passport requiring the need to go through a confirmation of citizenship procedure as a prerequisite for passport application eligibility.

Pleas arrive on time for your appointment. If possible, bring copies of your documents.

Fees will only be accepted in the form of a money order or cashier's check, which should be made payable to the Polish Consulate in Houston.

Passport fees:
$129 – passport for an adult
$42   – passport for a child under 12 years of age
$65   – passport for a child over 12 years of age
$65   – students between 18 and 26 years of age
$0     – passport for persons over 70 years of age
$18   – temporary passport issued while awaiting delivery of a biometric passport
Persons requesting the document to be sent FedEx pay an additional fee of $15.

Please note that we accept photographs that meet the requirements for US passports.
