Announcement No. 13 of the Minister of Health on the recognition of vaccination against COVID-19 carried out outside the Republic of Poland and the issuance of the EU COVID-19 Certificate (UCC)

Please be advised that every citizen of the Republic of Poland and foreigner entitled to participate in the National Immunization Program, who received vaccination against COVID-19 (one or two doses) outside the country, with a vaccine recognized by EU (Pfizer / BioNTech, Moderna, AstraZeneka and Johnson & Johnson), can register this vaccination in a National Vaccination Registration system maintained in the e-Health system (P1) by reporting it in a selected vaccination points.
To recognize a vaccine taken abroad in the e-health system, the following documents must be submitted:
1. the original with a copy of the document (certificate) proof of vaccination against COVID-19 from abroad, in accordance with the model in force in the country of receipt of vaccination;
2. sworn translation into Polish of the document referred to in point 1;
3. declarations of the person concerned or their legal representative containing consent to the processing of personal data related to the introduction of vaccination carried out abroad to the national system, including consent to the automatic generation of the EU COVID-19 Certificate in the e-Health (P1) system after the completion of the full vaccination cycle.
