Consent to issue a passport for a minor

1.   To obtain a passport/temporary passport for a minor child or ward, the consent of the mother and father or legal guardian or guardian of the minor is generally required​​​​​.

2.   In case of a passport application submitted in person by both parents, consent to the issuance of a passport document to the child is expressed in front of an official, using a device that allows for the reproduction of a handwritten signature (SignaturePad).

REMEMBER! During a passport visit, parents must identify themselves with a valid identity document (passport or identity card).

3.   In a situation where only one parent (guardian) submits a passport application for a child, the other parent (guardian) must:

REMEMBER! You must always provide full and correct information about yourself and your child, including PESEL numbers, for consent.

Exceptions:

  • Consent is not required from a mother or father who has been deprived of parental authority or whose parental authority has been suspended or limited in terms of expressing consent to the issuance of a passport document by a family court.

REMEMBER! In such a case, you must submit the original of the decision of the competent family court confirming that the other parent has been completely deprived of parental authority or that this authority has been suspended or limited in terms of expressing consent to the issuance of a passport document for the child. Remember that it is not enough for the family court to rule that the child should live with a given parent.

  • In the event of the death of one of the parents – the death certificate of the parent must be submitted.

  • In the event of undetermined paternity – a current copy of the child's Polish full birth certificate must be submitted.
