The information desk provides information about:
- services available at the PIM MSWiA
- locations where diagnostic tests prescribed by your doctor are conducted
- the organization of the outpatient clinic
as well as
- providing information on how to access your medical records
- downloading and submitting an application for photocopies of your medical records
- accessing the e-registration (“e-Rejestracja”) online service
- facilitating payments for additional medical services
Collection of Death Certificates:
- Death certificates can be collected starting from the second day after notifying the family of the patient's passing. Collections can be made on weekdays between 8:00 am and 4:00 pm.
- To arrange a time for collecting the death certificate, kindly call +48 47 722 18 89.
- The staff member handling the issuance of the death certificate will also provide you with guidance on the subsequent steps involving the document.
