As a rule, applicants must submit visa applications, together with all necessary documents, in person at the Polish Embassy. Applications submitted by travel agencies on behalf of the applicant(s) will NOT be accepted!

To avoid waiting time due to high volume of applicants applying at the same time, we will accept application according to appointment schedule generated on-line by the e-konsulat only.

Do I have to apply in person?

Visa application form must be submitted in person. Diplomatic mission does not accept application forms sent via fax, regular mail or e-mail.

How to book an appointment?

You can book your appointment to apply for a visa via the e-konsulat system. You can book the appointment maximum 6 months before the date of your intended travel.

What documents do I need to submit?

PLEASE, PRINT ALL YOUR VISA-RELATED DOCUMENTS DOUBLE-SIDED! 

1. A visa application form filled via the e-konsulat system, printed and signed.
2.    A 3.5 x 4.5 cm colour photo. The photo must be:
a.    sharp, taken against white background and printed on a quality paper,
b.    newer than 6 months,
c.    taken en face, clearly showing the eyes and face from both sides from the top of the head to the top of the shoulders with the face covering 70-80% of the photo. The photo must be taken without any headwear.
3.    Passport issued within last ten years, valid for at least three months from the expected return date, with at least two blank pages for visas. If 

you have another valid passport, you should attach it to the visa application form.
4.    A copy of the passport page with personal data and a photo. 
5.    Applicants for a Schengen (uniform) visa for single or double entries shall demonstrate that they have adequate and valid travel medical insurance covering any expenses that may arise during their stay/s in the territory of the Member States in case of a need to return for medical reasons, the need for urgent medical care, emergency inpatient treatment or death. Applicants for multiple-entry visas shall demonstrate that they have an adequate and valid travel medical insurance for the period of their first intended stay. The insurance must be valid throughout the whole territory of the Schengen area and cover the entire period of the planned stay. The minimum amount of insurance is EUR 30,000.
6.    Copy of an ID document to confirm residence within the consular district of your application.
7.    Proof that you have enough means of subsistence in the form decided by the mission:
a.    A valid bank account statement issued by your bank,
b.    A valid credit card with a certificate of means available,
c.    A valid income certificate from your employer for the last three months,
8.    A proof of accommodation (for example, an official invitation or a hotel booking or reservation).
9.    A document to confirm the purpose and the conditions of your planned stay in Poland.

List of supporting documents to be presented by visa applicants in Indonesia

I. General requirements

1. Proof of financial means (one or more of the following documents):

  • Recent personal bank statement or balance over the last three months
  • International credit card with a photocopy of the applicant’s bank statements associated to that credit card.

2. Proof of socio-economic situation (one or more of the following documents):

  • Job letter indicating the duration of recruitment, responsibility and salary
  • Copy of labour contract
  • Proof of social ties: Kartu Keluarga (family card), birth certificate, etc.

3. Plane ticket: copy of the flight reservation and travel itinerary.

4. Evidence of travel itineraries if visit in several Member States are planned. Confirmation of the reservation of an organised trip or any other appropriate document indicating the envisaged travel plans.

5. Minors (person under 18 travelling alone or with only one parent):

  • An authorisation to travel (alone or with one parent) indicating the purpose of the trip and signed by both parents/legal guardians or from the parent/guardian who does not travel, with copy of their passports or national identification cards. The signature(s) should be legalised by a notary office, or
  • the parents/legal guardians may lodge the application and authorisation in person;
  • the minor’s birth certificate (photocopy).

II. Documents to be submitted depending on the purpose of the trip

1. Private visit

  • In case of stay at a private address of family or a friend: written and signed invitation.
  • Proof of sponsorship, accommodation commitment from the host and/or private accommodation by means of an official national form,

2. Business trip

  • Letter of the Indonesian company introducing the businessperson, purpose and duration of the visit(s), financial cover of the visit, short company profile.
  • Invitation from the company that will be visited. The letter should incorporate relevant personal data of the invited, length of the invitation, reason and activity to develop in the Member State during the invitation.
  • If the purpose of the visit is to attend commercial fairs or congress, letters of invitation from the organisation of this events or invitations to attend them.
  • Any appropriate document indicating the accommodation envisaged or proof of sufficient means to cover the accommodation.

3. Tourism

  • Confirmation of the reservation of a organised trip, or
  • If the trip is selforganised: a complete travel itinerary, and confirmed hotel booking and/or proof of payment.

4. Trips for political, scientific or cultural reasons, to attend sport events or for religious purposes

  • Letter of invitation from the inviting entity in the Member State of destination in which it is stated the name of the organisation, contact data, data of the invited person, dates of stay in the Schengen area and reasons of the invitation.

5. Medical treatment

  • Letter from a physician or a hospital in the Schengen area confirming the appointment; and confirming as well the necessity of treatment at that hospital or clinic, and
  • Proof of medical insurance or proof of payment for medical treatment. The applicant must present documents that proof that he has enough economic means to pay the medical treatment in its country of destination.

6. Transit

  • In case of transit through the territory of the Member States, the applicant shall present the visa of the country to be visited after the transit, as well as the reservation of the airplane or train tickets towards its final destination.

7. Transit of seafarers

  • Seaman's book, with a minimum validity of 6 months, if relevant.
  • Information (or letter) from the maritime agency of the Member State where the seafarer will embark. The letter must be signed and stamped by that agency and include the following data:
  • Personal data: seafarer’s name and family name, place and date of birth, passport number (date of issue and date of expiry),
  • the seafarer’s position on the ship (if there are several sailors, this information may be included in an attached signed and stamped list),
  • name of the ship,
  • full itinerary:
  • date and airport of entry into the territory of the Member States,
  • port of embarking,
  • duration of the seafaerer’s stay on the boat,
  • port of disembarking, if applicable,
  • date of return to Indonesia, if applicable.

 

The maritime agency shall also indicate the name and address of the Indonesian agency that it collaborates with, which will be in charge of submitting the visa application or, that will be in charge of transporting the seafarer to the seaport upon arrival in the Member State where he or she is to board.

  • If the visa application is to be submitted by an Indonesian maritime agency: letter issued by the Indonesian agency that confirms the seafarer(s) employment on the ship.
  • The seafarer’s work contract (photocopy).

Additionally, when applying for a visa for a minor you should present:

  • Written consent of the child’s parent(s) to apply for a visa, confirmed by a notary. If there is only one guardian, it should be proved by a birth certificate, a court decision on exclusive parental custody or a death certificate of the other parent.
  • Originals and copies of parents' passports.
  • Original and a copy of a birth certificate.

If a child is entered in their parent's or a legal guardian’s passport, please submit a separate visa application. The visa will be pasted into the parent's or the legal guardian's passport.
Please note:

  • You must apply for a visa in a consular office or the embassy’s consular department in the consular district of your place of residence.
  • Usually, the above documents are enough to receive a visa, however, the consul may require additional documents.
  • Apply for a visa not earlier than three months before your planned journey.
  • The consul may, but does not have to, invite the applicant for an interview.
  • You may be banned from entering any of the Schengen States if you present forged documents or provide false information.
  • Receiving the visa does not guarantee that you will enter the Schengen area - the final decision is always made by the authorities of the country where you intend to cross the Schengen area border.

The Visa Information System (VIS) is used, among others, to exchange data between Member States on visa applications and visa-related decisions. Due to the fact that VIS has achieved its full operational efficiency, the travel document of a foreigner applying for a Schengen visa is not stamped with a stamp confirming the admissibility of the application referred to in art. 20 of Regulation (EC) No 810/2009 of the European Parliament and of the Council of 13 July 2009 establishing a Community Code on Visas (Visa Code).

How much does it cost?

The visa fee is according to the consular fees table.

Accepted forms of payment: cash.

The visa application fee is non-returnable, whatever the consul's decision. 

What is the waiting time?

The consul will make the decision on the visa within 15 calendar days. 

Pursuant to Art. 23 sec. 2 of the Visa Code, the period of processing of a Schengen visa application may be extended up to a maximum of 45 days.

For citizens of certain countries or categories of travelers, the period required to make a decision on the application may be extended due to the need for visa consultations.

How to appeal?

You can submit an appeal at the consulate every working day. 

Revocation or annulment of Schengen visa


If you have received a decision to revoke or annul your Schengen visa that you do not agree with, you have the right to ask for a reconsideration. However, if you have applied yourself for your Schengen visa to be revoked and the consul has agreed, you are not entitled to appeal.

You should apply for reconsideration within 14 days of receiving the decision to annul or revoke your Schengen visa at the consular post that issued the decision.

If you apply for reconsideration after your Schengen visa was revoked or annulled, you do not have to pay the consular fee.

Complaint about refusal to issue Schengen visa, decision to revoke or annul Schengen visa and the consul's activities in this respect.


If the consul who examines your reconsideration request, issues a new decision to refuse, annul or revoke your visa, which you do not agree with, you have the right to submit a complaint to the Provincial Administrative Court in Warsaw. The complaint should be forwarded through the consul who made the decision, within 30 days of receiving the decision taken as a result of the re-examination .


You do not pay a consular fee for submitting a complaint to the consul, but you have to pay fees for proceedings before administrative courts. Therefore, you have to note that the court will ask you to pay the court fee.
