Where to apply?
If your passport/temporary passport got lost or damaged or your personal data in it were used in an unauthorized way outside Poland, report it immediately at a Polish consular post.
Do I have to apply in person?
To report a problem with your passport, go to the nearest consular post or a consular post of your choice in office hours or
report it online—to do so you must have a trusted profile or an e-ID which allow you to confirm your identity online. Check how to create a trusted profile or how to use an e-ID or
make a report in writing provided that it contains your signature authenticated by a notary.
You can also report a lost or damaged passport of your child or a person over whom you have legal guardianship at the passport office.
REMEMBER! Honorary consuls do not accept reports of lost passports.
How to book an appointment?
You do not have to book an appointment.
What documents do I need to submit?
The consul will accept your report at the consular post.
A consular employee will fill in the electronic form, and you will confirm the accuracy of the data by signing on the designated device (Signature Pad).
As of the day of the report your passport becomes invalid.
REMEMBER! If found, the passport which has been reported to the consul as lost can no longer be used. You cannot withdraw your report of lost passport. If you find your passport or temporary passport, you must report it to the competent local passport authority (to a provincial governor in Poland or a consul when outside Poland).

What can I do if I lose or damage my passport outside Poland and need to urgently return to my place of permanent residence?
After you report a lost or damaged passport while staying outside Poland, upon your request and after checking your identity and citizenship, the consul can issue a temporary passport for you to return to your place of permanent residence (in Poland or in another country).
REMEMBER! Honorary consuls do not issue passports.
