There is no dedicated asylum application form, but an application for asylum must include:

• personal details of the applicant and the person on whose behalf the applicant is acting, sufficient for the conduct of the asylum procedure, i.e. at least the following details: full name, date and place of birth,
• an indication of the country of origin of the applicant and the person on whose behalf the applicant is acting,
• a description of the relevant events that constitute the grounds for the asylum application.

The application must be submitted directly to the Head of the Office for Foreigners. Apart from the applicant, the application may also cover other persons, in particular the applicant’s spouse or minor children. The applicant’s spouse must give his/her written consent to the lodging of an application for asylum on the spouse’s behalf and on behalf of the spouse’s minor children. This consent constitutes a power of attorney authorising the applicant to act on behalf of those persons.

An application for asylum may be lodged while staying in the territory of the Republic of Poland or while abroad.

An application for asylum may be:

• sent by post to the following address: Office for Foreigners, ul. Taborowa 33 02-699 Warsaw, or
• filed in person at the Office for Foreigners’ filing office, at ul. Taborowa 33 in Warsaw during opening hours; or
• sent to the electronic delivery address: AE:PL-63297-42869-TJTIE-23.

An application for asylum cannot be submitted by e-mail, as this form of correspondence has no legal effect.

A foreigner who is applying for asylum while staying in the territory of the Republic of Poland is required to have his/her fingerprints taken and to be photographed. Taking the fingerprints and photographs is the responsibility of the commander of the Border Guard unit whose territorial jurisdiction covers the capital city of Warsaw. The date and place of these procedures will be communicated to the foreigner in writing. If the foreigner fails to follow these routines, his/her application for asylum will be left unexamined.

An application for asylum may be:

• sent by post to the following address: Office for Foreigners, ul. Taborowa 33 02-699 Warsaw, or
• sent to the electronic delivery address: AE:PL-63297-42869-TJTIE-23., or
• lodged through an authorised representative, or
• lodged through a consul of the Republic of Poland in the applicant’s country of stay.

A foreigner who is applying for asylum while abroad is required to attach a photograph to the application and — upon arrival in the Republic of Poland on a national visa in order to participate in the asylum procedure or to be granted asylum — to have his/her fingerprints taken. If the foreigner fails to follow these routines, his/her application for asylum will be left unexamined.
