PLEASE NOTE!
New-borns are not covered by medical care until their parents file the “request for including their child in the procedure”. The application must be filed immediately, best on the day of the child’s birth at the headquarters of Office for Foreigners in Warsaw at ul. Taborowa 33 Immediate inclusion of the new-born to the procedure by the parents allows the medical operator to settle the costs of birth and subsequent medical care for the child. At the same time, please remember about the need to provide the application along with an attached child’s birth certificate, obtained from the Registrar’s Office.
