Where to apply?

If you are abroad, report a lost passport or temporary passport immediately in a Polish consular office.

Filing a lost passport report at the consulate means that your document becomes invalid on the day when you report its loss or damage to the consul.

Do I have to apply in person?

In order to report a lost passport or temporary passport, go to the nearest or selected consular office.

NOTE: You do not need to report a lost passport personally in the consulate if you have a trusted profile (Profil Zaufany), qualified certificate or e-identity card (eDowód). In such cases, you can report the loss or damage of your passport document either in writing using an electronic form or online via the provided electronic service. In addition, you can also report a lost passport in writing with your signature certified by a notary.

How to book an appointment?

If you want to report a lost passport at the Polish consulate, you do not need to make an appointment via the eKonsulat booking system.

What documents do I need to submit?

Filing a lost passport report at the consulate is done electronically at the consular office. A consular employee will file your application and present it to you for signing on a dedicated device (signature pad).

REMEMBER!

  • If you find your passport document after reporting its loss to the consul, you can no longer use it or withdraw the lost passport report.
  • A found passport or temporary passport should be returned to the Polish consular office. You cannot receive the passport back.
  • You must return the passport regardless of whether you already have a replacement document or you have not yet applied for one.
  • Any attempts to use a reported document to cross international borders may result in serious consequences and hindrances.

What should I do if my passport document is lost or destroyed and I need to urgently return to my place of residence?

If you lose your passport document abroad, the consul will, on your request and after confirming your identity and citizenship, issue to you a temporary passport allowing you to return to your place of residence (Poland or elsewhere).

Remember that an honorary consul cannot issue temporary passports nor file lost passport reports.
