To apply for a passport, you need to present:
- previous Polish passport (in case of the first Polish passport – decision confirming the Polish citizenship and Polish birth certificate and Polish marriage certificate (if applicable)
- other valid proof of identity, e.g. South African passport (if the Polish passport is no longer valid)
- 1 colour photo (not older than 6 months, on a light background, the face should occupy 70-80% of the photo)
- marriage certificate registered in Poland (in the case of a change of name in the passport)
- child's birth certificate (in the case of applying for a passport for a minor child). Presence of both parents is mandatory.
- fee of ZAR 2239 in cash (persons over 70 years of age - no fee, children below 12 – 713 ZAR, 50% discount - only after documenting ongoing studies or pension entitlements from Poland).
