Please note that to submit an application, you must present:
- a previous Polish passport, 
- another valid ID, e.g., a South African passport (if your Polish passport is no longer valid),
- 1 colour photo (no older than 6 months, against a light background, with your face occupying 70-80% of the photo),
- a marriage certificate registered in Poland (if you have changed your name on your passport),
- a child's birth certificate (if you are applying for a passport for a minor child)
- a cash fee of 2,239 Rand (for those over 70 years of age - no fee, and 50% discount only upon proof of ongoing studies or pension entitlements from Poland)
