Reporting the list of transport means

Business entities licensed to conduct wholesale trade in medicinal products, to manufacture or import a medicinal product are required to provide the Chief Pharmaceutical Inspector with information on the means of transport used for the business activities covered by the licence.
 
Business entities are required to provide a full and up-to-date list of transport means in accordance with the following guidelines:

Who is subject to this obligation?

  • Manufacturers and importers of medicinal products (Article 42(1)(4)(c) of the Pharmaceutical Law Act);
  • Pharmaceutical wholesalers (Article 78(1)(15) of the Pharmaceutical Law Act).

What types of data are to be provided to the Chief Pharmaceutical Inspector?

  • business details (name, NIP [Tax Identification Number]);
  • full, up-to-date list of transport means, i.e. make and licence plate number of the vehicle used for the business activities covered by the licence, while in the case of using railroad transport – carrier details.

How to file the report?

The report is filed by sending the following documents to transport@gif.gov.pl:

  • a filled-in editable form (file template provided below);

and

  • a scan of the form filled-in and signed by a person authorised to represent the business entity.

How to update the report?

If previously provided data are subject to change, the up-to-date list of transport means is to be sent to transport@gif.gov.pl based on the same procedure that applied to the previous list.
